Patient Chart Lab Report Sheets
Mount sheets are usually 8-1/2″ x 11″ cut sheet forms that are manufactured with transfer tape on them and are designed to collect and organize patient information. When the liner is removed, the adhesive is exposed so that lab report (urology, hematology, etc.) data can be attached. Mount sheets are typically drilled with five holes at the top (standard hospital punch pattern) to allow the forms to be secured inside the patient’s chart. Mount sheets are used widely in a number of medical facilities including hospitals, medical clinics, out-patient surgery centers, urgent care centers, and rehabilitation centers.
