Patient sign in sheets are made with a special liner page that has peelable labels to remove patient information from the front, and transfers data to a back page to provide a full record of patient visits for the day, while not allowing any of the data to show through the liner. Measuring 8.5" x 11" in size, they include printed fields for name, arrival time, appointment time, appointment with, new patient check box, and change of address, phone and insurance since the last visit. They are also HIPPA compliant. Each pack contains 125 forms.
