Protecting Patient Information
Our patient sign-in sheets feature a removable label that assists in protecting patient information. Once the patient signs in, the labels can then be applied to the chart to record the visit. Those same labels correspond to a number slip that also tears off from the form. After the patient sign-in form has been completely filled, a copy of the completed sheet is available behind special backing paper that gives the office a complete daily patient log that is only seen by office staff. Other patients signing in don’t see personal information that other guests have entered onto the sheet. They simply sign in on the next available line under where the last label has been removed.
All information requested to be completed on the sign-in sheet conforms to HIPAA regulations. That means that the sheets do not display medical information in a public facing manner. Information that can be collected is the date, patient’s name, arrival time, appointment time and who the appointment is with.
